This is a consent form made by Alysia and I stating the circumstances and asking for the cast members to sign in agreement that they will be in our music video and follow the rules and regulations. The form underneath shows the consent form without the signed permission.

The images to the right of the blog post show the cast members have signed the agreement to be apart of our music video. By them signing the agreement they have to make sure they always come to every shooting day and on time, always be prepared to do anything and also that they can not give any idea's in.
